A person have orothostatic hypotension, history of creatinine3.2 with microalbuminuria,nausea and dry heaving 24hours,reduced urine output,reduced urine output, reduced appetite x3,nausea and vomiting x 3to 4 days ,tachycardia,lightheadedness when stands up quickly, new NSAID use,fatigue, dry mucousmembrane, mild tenderness in epigastric and periumbilical region .
Mr. Fowler is 70 yrs old who is sent to the ED derby his primarycare physicians for further evaluation .
