Each journal entry should combine description, analysis, and reflection to comprehensively view your experiences. 

Instructions Reflect on your clinical experiences in the medical-surgical nursing unit. • Reflect on your experiences during your preceptorship rotation. For each entry, describe the situation, analyze your actions and their outcomes, and reflect on what you learned and how you could improve. • Write a journal entry for each prompt below. 96 year old female actively dying. Pt was kept alive intubated. Family at bedside the whole time tx provided. Family decided put pt on comfort measures. Pt was extubated and given ativan and lorazepam for comfort. Family activly crying. Asked family if their needs were being met and provided compassion. Family was wanting questions from nurse I’m precepting with on what to do. Asking what we would do, if this was their mother, prior to extubating. Advised could not provided answer. Offered for patient family to speak with Dr.

Preceptor had their own patients but the other RN who worked closely with her on most days they helped one another and tag teamed their pt’s with team work

When providing post mortum care advised er tech to use warm wipes to clean pt. Advised will help get dry fecal matter off pt. Also, think of what treatment we would provide a pt who was alive. Do not think of them as deceased. ER tech was receptive to education
