Summary of strengths:

Empathy is one of my greatest strengths and is one of the most needed skills in the nursing field. I intended to utilize this skill alongside an active listening skill to give the utmost care the patient deserves. Also, as an NP student, I understand the need to seek help from my preceptor when needed and demonstrate professionalism at all times as I work towards being a competent clinician.

1 Goal I will be able to recognize specific signs and symptoms associated with certain mental health issues by the end of this quarter

a.       objective: I will be able to list up to 10 potential diagnoses with their reasons at the end of this quarter

b.      Objective: I will be able to confidently put together a list of potential diagnoses for each client by the end of the quarter

Opportunities for growth:

Some of the areas I’m looking for growth in the next ten weeks of the practicum will include working with my preceptor and other treatment teams in learning the prominent signs and symptoms of psychiatric diagnosis and Indications for prescribing specific medications for mental health patients. I would also love to know more about handling and addressing the group and family dynamics and individual personalities.

c.     Objective: I will observe and have my preceptor explain to me how the Patient’s potential diagnoses are chosen and concluded

Goal: My goal is to work with my preceptor and the rest of the treatment team for the best outcome for the patient’s health in this quarter

a.       Objective: To assess and reassess the patients as needed for a positive outcome.

b.      Objective: To modify patient’s treatment as needed for a positive result

c.       Objective: To assess the patient’s reaction to care appropriately and make a choice on how to alter the care by using the pt’s information

Goal: My goal is to improve my communication skill and increase my attentiveness to the patient’s concerns by the end of semester.
