Case Study Assessment

Students will write a paper of a case assessment for the online
class. Students may use a case of their choice. If this is a real case, disguise the name of the
client for confidentiality purposes. Use the case study to prepare a role play, initial case
assessment, and service delivery plan for the client. As a reminder, you will use these items to
prepare a video recording and a paper. The video recording should consist of you acting in the
role of the human services worker and another individual serving in the role of the client. The
student will record a role play consisting of the interview, initial case assessment, and service
delivery plan to a treatment team of their classmates.
Be aware of recording, organizing, and assessing the relevance, adequacy, accuracy, and
validity of information provided by the client in your assessment. The assessment should
include a contract with measurable goals and objectives for implementation of a plan of action
based upon the needs of the client. The paper should focus on four areas; the client’s
background, problem, goals, and provide the (academic) rational for selected goals i.e. how did
the student determine the goals.
This written portion of the assignment should be typed, double-spaced, and follow APA
formatting guidelines (see Purdue Owl). The paper should have a minimum of a cover page, an
opening paragraph, level-one headers, citations, a conclusion section, and a reference page
with references. Use the rubric for additional material to consider for the paper. The paper
should not exceed 10 pages. Students should consider the issues affecting the case such as
gender, race, age, disability, or other culturally diverse characteristics.
