You need a disease that has affected two cranial nerves that are described in the table and the rest of the cranial nerves that are normal. The teacher does not want them to only put normal but to describe that cranial nerve showing that it is fine. You do not have to fill out page 5, the evaluation is only from pages 1 to 4.
