Statement of purpose goals and smart objectives
Start this section with: “Based on my findings and recommendations, the purpose of my program will be to [Healthy People 2030 Objective] in [target demographic/population] by targeting [specific behavior/risk factors]”. Fill in the blanks with your program’s information. Then describe the goals and objectives of your program in the following format (one sentence maximum for each of the following): One page minimum
- SMART Overall Program Goal: This is your Healthy People 2030 Objective plus target demographic and location.
- SMART Process Objective: The process objective is based on administrative tasks of the program (i.e., # of classes held, “very good” overall rating on satisfaction surveys, # of sites contacted for participation)
- SMART Impact Learner Objective: The impact learner objective is based on what you want your target audience to learn (i.e., changes in knowledge, attitude, and beliefs). This typically happens immediately upon participant interaction with the intervention/program. This is based on Phase 3 Educational & Ecological Assessment of the PRECEDE-PROCEED Model.
- SMART Impact Behavioral Objective: The impact behavior objectives is based on what you want your target audience to do based on the information they receive (i.e., actions, habits, cessation, etc.). This typically happens withing three (3) to six (6) months of intervention/program’s implementation and is based on Phase 2b Epidemiological Assessment – Behavioral & Environmental of the PRECEDE-PROCEED Model.
- SMART Outcome Objective: The outcome objective happens after the intervention or program has finished, typically within one (1) to ten (10) years and is based on Phase 2a Epidemiological Assessment. This must related directly back to the overall program goal.
Evidence based activity proposal and explanation One Page minimumIn this section you will expand on and describe what evidence-based practices you would select as your intervention by referencing your research and specific program goals in sections 4-6 of this paper. Thoroughly explain how these practices/activities will aid you in achieving your proposed program goals and objectives.
If you are using a program modeled after a current or past program make sure to cite the source of the program. Provide specific examples of how the practice is evidence-based, meaning it has been tested and proven to work. Refer to section 4 of your proposal.

Using your proposal in section 7, state what specific practices/activities you will use to address each concept of the theory to help achieve your objectives and cite examples of what will be presented (i.e., skills-training, one-on-one consultation, cessation class) that has been proven to work and/or was recommended from the literature in your needs assessment, and a specific example of what content you will present.
What specific activities will need to take place for this program/intervention to be effective? The practice/activity (i.e., class, skills training, etc.) must be described in enough detail for someone else to carry it out. You must include specific activities, materials needed, time needed, and step-by-step instructions.
