Imagine you are a community health nurse assigned to care for a family with a newly diagnosed type-2 diabetic member. The diabetic family member, JK, is a 66-year-old African American woman with hypertension and asthma. JK lives in a food desert and does not drive. She does not have a computer, but does have a cell phone with limited data. She has a very low limit credit card that she does not like to use and is fearful of using the card on her phone. JK’s family visits her weekly, but they have complicated lives and are unable to provide daily care for her. They take a bus to see her and do not have reliable transportation or finances to assist with her care.
JK lives in a community near you, you choose the community that interests you. Her budget will be based on the average senior income on Social Security and Medi-Caid. She does not have a savings after she lost her husband to diabetes and it drained her resources. He was self employed and did not have a retirement. She prefers to pay with cash for her needs.
