Was just going thru MyEvaluations logs this morning, and noticed most of yours had “New OB visit” as the type of encounter. I changed them to “acute/episodic care” as I assumed they were not new OB visits based on your notes (and they were older patients with no mention of pregnancy). Most of the time you should have acute/episodic care or chronic illness management. I am attaching some notes to help you with documentation in MyEvaluations.
