Situation: The nurse doesn’t know how to use the new equipment to monitor the patient vital signs. She invented the vital signs of a patient who came out of surgery.

Situation: One patient was not given analgesics causing her pain to worsen. The nurse in charge was injecting them because she was addicted to painkillers
