Note Course Description (family nurse Practioner primary health of chronic illness across the lifespan-clinical practicum?

The 135 hours of precepted clinical practice time is focused on acquiring the knowledge and skills needed to assess, maintain, and promote the health and well-being of culturally diverse population with chronic conditions in a primary care settings, community clinics, and/or skilled nursing facility. Evaluates the evidence for screening, differential diagnosis, and management of health problems, including pharmacological and non-pharmacological treatment modalities in a culturally appropriate manner. healthcare includes assessment of health needs, diagnostic reasoning, the planning, implementation and management of care, and the evaluation outcomes using the SOAP format for documentation. Evaluation is to include revision of the plan, consultation, and referrals as needed. Standardized procedures will be discussed and reviewed throughout the class. Clinical evaluations will be performed using the College Practicum evaluation Rubric. Project Concert will track the students’ clinical experiences.

 

Course Learning Outcomes

Upon completion of the course, the student will be able to:

  1. Perform levels of prevention and primary care management of chronic healthcare problems of diverse clients based on advanced practice nursing theory and evidence based practice with emphasis to resources allocations, efficacy, acceptability, simplicity, adherence, spirituality, and safety.
  2. Formulate plans for illness prevention and treatment, wellness promotion, and health problem management based on current scientific rationale and research,
  3. evidence-based practice guidelines and standards of care for clients with selected chronic health problems.
  4. Practice the advanced professional roles of clinician, consultant, collaborator, community advocate, educator and leader in the care of chronic healthcare problems.
  5. Discuss collaborative processes of the interdisciplinary healthcare team in facilitating the client’s progress toward maximum functional health.
  6. Integrate the effective and appropriate use of information and communication technologies to promote patient safety, providing high quality care within cost-effective services.
  7. Implement effective SOAP documentation of chronic healthcare problems across the lifespan and meet the standards of professional and organizational care, regulatory requirements, fiscal responsibility, and criteria for reimbursement.
  8. Practice consistent and accurate record-keeping.
