 Cases You work in a women’s health clinic. Abnormal uterine bleeding (AUB) is one of the most common reasons women seek health care. A new female patient, C.Y. 32-year-old patient consults your office because she presents hirsutism, clitoral hypertrophy and menstrual dysfunction. She also stated she has gain weight lately. You suspect of Polycystic Ovary Syndrome (PCOS). Create a focused SOAP note to support (only include positive and pertinent data) for a 40-year-old female patient with diagnosis of Endometriosis.
